Billing and Insurance Information
This practice is a fee for service practice. As a courtesy to all of our patients,
we will file requests for predetermination of benefits as necessary, as well as any
claims for completed services with all assignments of benefits going directly to the patient.
As we understand that insurance issues can be confusing, we will do what we can to assist you
in receiving the reimbursement for the treatment you received in our office. Due to the nature
of the services we provide and the number of insurance plans available, we cannot determine your
benefits for you.
You will be responsible for payment of services as they are rendered. Most often with the more complex
treatments, a down payment will be required to initiate treatment and the balance will be due at the completion
of treatment.
Our office has information on CareCredit, a financing company assist which assists patients in financing their complex
